Questions about CFSS
Community First Service and Supports (CFSS) is a new homecare program for Medicaid enrollees. It is replacing the Personal Choice Assistance (PCA) and the Consumer Support Grant (CSG) program.
The MN Department of Human Services designed the CFSS program to offer people more choice, control, and flexibility with the services and supports needed to live and work in the community.
All CFSS participants must select a consultation provider, such as Asura. Your Asura advisor will answer questions about the CFSS program and help you decide which service model best meets your needs. (612) 416-2291
CFSS provides assistance with activities like:
- Eating, bathing, grooming, and transferring (ADLs- Activities of Daily Living)
- Shopping, meals, and household tasks (IADLs- Instrumental Activities of Daily Living)
- Health-related activities like exercises and managing medications
- Observation and redirection of behaviors
- Acquiring independence skills
- Ensuring continuity of services
- Budget model
In the budget model, you employ your caregivers and pay them from a pre-set budget. You will be responsible for recruiting, hiring, training and supervising your caregivers and setting their hourly wage. You select a financial management service (FMS) provider to help you manage your budget and comply with all applicable laws. - Agency provider model
In the agency model, you select a homecare agency to serve as your caregiver’s employer. This means the agency will recruit, hire, train, supervise, and pay your caregiver. The agency will also determine the hourly wage of your caregiver.
CFSS will begin October 1st, 2024. After that date, new participants will begin using CFSS immediately, and people currently using PCA and CSG services will transition to CFSS when their annual service agreement is renewed.

Asura Homecare Advisors are CFSS consultation providers. Consultation services are required to educate people about CFSS and, the different service models; assists with writing service delivery plans; reviewing service plans; and providing ongoing support.
Participants must transition at their yearly reassessment to ensure no disruption of services

Yes, you will be able to continue working with the caregiver you know and trust.
No. The switch to CFSS will not impact your approved hours.
